Step 4: Choose any of the three options available on this form. The first option is deleting a person's name from the electoral roll due to certain reasons. Another reason is objecting to the proposed inclusion of a person's name due to some reason. Lastly, you can delete your name from the electoral roll due to other reasons which you need to state.

Step 5: Select whether you want to attach a death certificate or not.

Step 6: Provide details like name, surname, EPIC no. and address of a person against whom you want to raise an objection.

Step 7:  Read the declaration and enter the date, place and your signature.

Step 8: Submit this form along with any supporting documents if required, to the Electoral Registration Office. You can also courier it.

Step 9: You will receive the acknowledgement receipt for your application.
